If we do not address the issue to your satisfaction, you are encouraged to report a concern or register a complaint with the Joint Commission's Office of Quality Monitoring. You may do so by calling 1-800-994-6610, faxing 630-792-5636, sending an e-mail to complaint@jointcommission.org, or mailing a letter to the Office of Quality Monitoring, The Joint Commission, One Renaissance Boulevard, Oakbrook Terrace, IL  60181.
